We went on a last minute decision around Christmas Eve when they had limited opening hours and were able to get right in buying tickets at the door so I don’t think you need to book in advance. Maybe only if you expect a large crowd say, for example, during the busy summer months or something like that.
Is this a different entrance to Buckingham Palace ticket or is it an added extra within this visit, similar to when you visit a museum but have to pay extra for a specific exhibition. I'm thinking of buying one of those London Pass tickets & the Queens Gallery is included.

Its a separate entrance and fee but you can buy a ticket/pass for a year. Its well worth it as the exhibitions are great.
